As rumors continue to circulate concerning the validity of Alberto Del Rio's concussion status, as he did wrestle a match on Raw this week, The Wrestling Observer is reporting that Del Rio did in fact suffer a concussion, but was medically cleared to wrestle by the time No Way Out came around.
The reason why he didn't wrestle as advertised at the PPV is because WWE officials had no way of knowing, at the time Del Rio was diagnosed with the concussion, that he would be cleared to compete in time for the PPV. So they made the decision to replace him in the match with Dolph Ziggler.
It's also being said that WWE is taking concussions very seriously nowadays, and decided to err on the side of caution where Del Rio is concerned.
